Community engagement efforts show that 75% of survey respondents are either somewhat comfortable or not comfortable with contacting their elected officials. Reasons cited include not knowing contact information and feeling as if the elected officials either do not care or understand the issues. Respondents were asked for suggestions to help them feel more confident. Feedback included information on how to contact elected officials, actual contact information for elected officials, and opportunities to meet with them at planned community events. Aging and Disability Services will provide educational opportunities for county residents to help them feel more confident to advocate for themselves and others. By educating older adults to work together, they can have an even greater impact. This can help elected officials understand the needs of their constituents. Plan or strategy: We will create and keep current and “Advocacy” page on the county website. It will include information on basic Advocacy, local, state and Federal legislators, voting, and current issues. We will work with the GWAAR Advocacy & Public Policy Coordinator to host training sessions locally for purposes of assisting older adults overcome the lack of confidence they have in speaking with legislators and local officials. We will host opportunities for legislators and local officials to get to know their constituents and understand the issues they face in Bayfield County. DRAFT Strategy 1. Feedback from all community engagement efforts clearly indicated social isolaon and loneliness is of concern to and being experienced by many older adults. Survey results indicated that 44% of respondents live alone. Respondents would like to see more senior meals, more community meals, more social gatherings, more opportunies for games, and other acvies such as dancing, gardening, computer classes, craing, etc. Some even suggested locaons in which to hold acvies and they include libraries, senior centers, churches or clubs. To address the health effects of social isolaon and loneliness, we will (1) create a coalion of community representaves, agencies and county residents, (2) develop an awareness campaign, and (3) pilot a Repair Café in the Cable area. By creang a coalion, a group of non-governmental individuals will become educated and empowered by understanding what is at stake and what needs to be done to engage people. The awareness campaign will bring to light the need of awareness and acons that need to occur at the community or grass roots level. The pilot project will be a way to engage individuals around a parcular project, vs. a gathering or social event which can be hard for those who find it difficult to engage socially. DRAFT Plan or strategy: Due to a lack of staffing and financial resources, never has this amount of effort been able to be put toward social isolaon and loneliness. A grant has been secured by WIDHS, which allows for hiring a facilitator to idenfy partners invested in working on this issue, develop a coalion, host meengs, and create an acon plan. We will raise public awareness of loneliness as a public health issue and share strategies to improve connecons and create a feeling of purpose. We will develop awareness materials and conduct a social isolaon and loneliness campaign using social media, print, and radio and local outlets. The coalion will host at least 5 community gatherings in three communies for the purpose of increasing awareness of social connecons and create direct opportunies for individuals in Bayfield County to connect. We believe the increased meaningful connecons among older adults will reduce the health effects of loneliness. Strategy 1: Create a coalion of community members. Strategy 2: Raise public awareness by developing educaonal materials and resource lists and engage in a campaign using social media, print, and radio and local outlets. Geographically, Bayfield County is a large, rural county with pockets of older people scaered throughout. Plan survey results and conversaons with seniors show that seniors feel more socially isolated in the winter months and would like to aend a meal site closer to where they live. Adding permanent congregate meal sites is difficult because of lack of funding and food vendors. Food vendors may be willing to partner with us in less-busy winter months or to partner with us for special events. With this in mind, staff and the Nutrion Advisory Council (NAC) would like to explore different ways to provide congregate meal opportunies around the county to increase outreach, improve social connectedness, prevent malnutrion, and introduce the ADRC and Aging Unit services to new people. Addionally, partnering with other county and community organizaons will be explored to offer “more than just a meal” and give seniors added incenve to parcipate. Exploring different ideas and pilong them will provide informaon about how people want to parcipate in the congregate meal program and will honor what is valuable to them. If we combine meals with addional community resources, seniors will get more out of their experience. Obtaining feedback from parcipants about each type of congregate meal offered plus parcipaon data will let us know if seniors have benefited from these efforts. Plan or strategy: Strategy 1: Determine how to solicit informaon from older people who are eligible to aend congregate sites, but don’t currently parcipate. Strategy 2: Generate a list of ideas of other ways to provide congregate meals aer receiving input from seniors Strategy 3: Determine if there is enough funding to trial different ideas. If not, determine if advocang for addional funding is appropriate. Strategy 4:  Choose 2 new ideas to try during the plan period  Choose a community and site locaon to trial each idea  Develop a program to go with the meal type  Choose a vendor, secure volunteers and staff to provide meal service  Provide the meal Strategy 5: Based on feedback from the NAC, program staff and parcipants, determine how successful each effort was related to benefit to parcipants vs. cost and effort to organize. Community engagement efforts found that 28% of respondents care and/or support another person and that 60% of the care recipients were older adults. Addionally, the survey shows that when asked to rank issues that were most important for older adults, support for caregivers was ranked 55% as “more important”. When asked what would be helpful to caregivers, the top 4 concepts included: advocacy for care recipient and healthcare issues, opportunies for self-care, a caregiver resource guide and financial assistance. To address the needs of caregivers, we will expand opons so they will have increased opportunies to support their caregiving roles. We will provide caregiver specific advocacy training, explore opportunies for self-care, create a specific caregiver resource guide and advocate for creave ways for caregivers to hire non-professional support. Plan or strategy: We will incorporate a specific track of advocacy training for caregivers when focusing on hosng general advocacy training for older adults. Seflf-care is an important aspect for a caregiver to be their best and remain healthy. We will research and gather ideas on what will be appropriate ways to encourage self-care. We will determine how the NFCSP grant can help to meet this need. We will then create a lisng of self-care ideas for caregivers DRAFT Due to the lack of professional caregivers available for hire, it is imperave we become creave is supporng caregivers in more creave ways. Bayfield County has a Meal Priorizaon policy and a Waing List policy. Both need to be updated to reflect new informaon and resources available. Staff reorientaon and orientaon regarding both processes is also necessary. By ulizing the GWAAR Person- Centered Meal Priorizaon Process, we can implement a standardized process to assess individual needs and offer resources to best meet personal goals. Plan or strategy: Staff will implement the GWAAR Person-Centered Meal Priorizaon Process to opmize the delivery of meals and related services within the OAA Nutrion Program. This will ensure that individuals with the highest nutrional needs are being served within the means of program resources while also offering person-centered services to those with moderate to low needs that meet the individual where they are to opmize their nutrion status and enhance their quality of life. Nutrion resource opons will be tailored to meet individual needs, providing appropriate nutrion and supporve services that are grounded in person-centered pracces and equitable to all by ensuring transportaon needs and financial burdens are met. Strategy 1. Update local Meal Priorizaon policy and Waing List policy to reflect and be consistent with the GWAAR Person-Centered Meal Priorizaon Process. Strategy 2. Staff have worked hard to implement programming that is of interest and benefit to older adults in Bayfield County. We currently offer Tai Ji Quan: Moving for Beer Balance, Powerful Tools for Caregivers, Strong Bodies and Mind Over Maer. Both Strong Bodies and Mind Over Maer were implemented in 2024 using IIID ARPA funding to get the programs up and running. These programs are being led by 8 different community volunteers who have invested in these programs and believe in their value for older adults. Programs are currently being offered in 8 different communies through Bayfield County. With expanding and creang programming, we have had growing pains. We must take me to improve current procedures, implement consistent outreach pracces, collect appropriate data and ensure consumer sasfacon. This will allow for streamlined pracces and improved experiences. Plan or strategy: Current outreach efforts are minimal and not tracked. We will review current pracces, record what is being done, and determine what other techniques can be added. By doing this, we can understand where we may increase efforts to reach those more prone to isolaon, such as caregivers, homebound parcipants and rural areas of the county. Based on informaon gathered, we will look for ways to market programs to external partners, such as health care facilies.
