IEP Meetings – case managers at a family’s request can be part of meetings to help advocate

COMMITTEE MEETING MINUTES
Bayfield County Courthouse Conference Room A &
Remote via Teams Meeting or Phone
Tuesday, July 9, 2024, at 10:00 a.m.
Committee Members Present: Hayley Weyers, Michelle Lampson, Anna Hanson and Sherri Paulson
(by Teams).
Others Present: Liv Garramone (BC CST Case Manager), and Heather Gilbertson (BC Support Staff),
Mary Anich (BC Manager)
Call to order and Introductions: Weyers called the meeting to order at 10:08 and introductions
were made.
Discussion and Possible Action - Review/adoption of minutes from May 14, 2024, meeting:
Motion by Lampson, seconded by Hanson to approve the May 14, 2024,
meeting minutes as is. All were in favor, motion carried.
Guest Presentation Series- Liv Garramone, Coordinated Services Team Specialist.
CST is a new program for clients. Clients need to be involved in two systems of care, live in the
community and have risk involvement.
Garramone and Anich have spent the last 4-6 months working with the State Contact for
additional direction in building the program and are hoping to take on clients in October.
CST is a collaborative team approach program, and the team works with the client in the least
restrictive setting. The team is made up of the parents of the youth, school staff, relatives, friends
of the family, and therapists. The team helps to carry out coordinated services.
The team meets and develops a plan of care and a crisis plan if needed. Clients and parents can
edit the plan whenever needed.
The goal of the CST program is to keep the child in the home and community.
Program Participants: Enrollment Report: The CLTS/CCOP program has 47 active enrollments and 11
active referrals.
Since the agenda was sent out the numbers have changed to 48 active enrollments and 10 referrals.
Weyers stated there is a lot of need in Bayfield County for the CLTS program which is why the program is
constantly growing.
Coordinated Services Team Participation Report: Future reports
Other Informational Items:
Multiple clients have attended summer camp this year.
Camp Avanti is run once a year in the summer, it consists of a collection of professional therapists
who volunteer their time to run the camp. Avanti Camp is based in Sawyer County.
