Standing Note: Topics and discussions included in this meeting are specific to the overall quality of the CCS program,
and programmatic-level input and feedback that inform it. Examples include review of the CCS plan and policies
applicable to how services impact all CCS participants. Individual grievances, staffing, or other concerns not applicable to
all participants should be addressed through the most appropriate alternative channels based on policy.

i. Sandra Notzke (BCCS Vice Chairperson / Participant Member) resignation effective 4/14/25.
ii. Due to the restructuring into one regional committee,
1. Jan Kupczyk (Vice Chairperson / Ashland Community Member) will now transition into the
Vice Chair for the regional committee.
2. Thomas Mittelstaedt (Chairperson / Bayfield Participant Member) will now transition into
the Chair of the regional committee.
3. Member at Large roles are no longer relevant.
4. All member term appointments renewed as of today
5. Bayfield County Representative change.
iii. Verification of committee member positions (as listed above)
1. Process Reminder: If members above do not wish to continue with their current roles,
new members must be appointed and voted on for chair and vice chair roles.

4. CCS Policy Updates
A. Committee Restructuring
i. Per a mutually agreed decision by Ashland and Bayfield Counties; the CCS Coordination
Committees will no longer function as separate Ashland and Bayfield committees.
ii. Effective as of today, all Coordination Committee members will now be considered part of one
Regional Comprehensive Community Services (CCS) Coordination Committee, representing both
counties.
iii. Per the updated Coordination Committee policy, all efforts will still be made to ensure there is
equal representation for each county within the regional committee members.
iv. Meeting notification process update: Draft agenda will be sent / posted one week before the
meeting date. Updates to the draft agenda must be submitted within two days.

B. Quarterly Plan & Policy updates / changes:
i. Coordination Committee Policy
1. adjusted to reflect new format of one regional committee and specified processes for
committee appointments, which differ between counties.

ii. Discharge Policy
1. adjusted to reflect additional potential reasons for discharge; lack of rehabilitation
determined by MHP & no meaningful contact with CCS team members.
iii. Ethics and Boundaries Policy
1. New policy.
iv. Initial Orientation and Training Policy for CCS Providers
1. adjusted to reflect change in timeline for orientation trainings: DSPS licensed / certified
providers can use trainings completed up to two years prior to CCS start date if relevant
to orientation topics.
v. No-Show and Service Engagement Policy
1. this policy was previously reviewed with the committee, but was inadvertently left out of
the CCS Plan when most recent updates were made. New content includes: policy title
updated, specified SF intervention also applies when contact with the participant is
made, but meaningful engagement is not established / maintained, procedure for follow
up if meaningful engagement is not made for a full month.
vi. Vendor Listing
1. adjustments: new vendors added, exited providers deleted, contact information changes
updated.
vii. CCS Regional Quality Improvement Plan
1. adjusted to reflect regional committee roster will be used as data source opposed to
ACCS vs BCCS rosters.
