Sanitary Permit issuance, if required, needs to occur prior to Land Use Permit issuance. Failure to obtain a permit or starting construction without a permit will result in penalties. The local Town, Village, City, State or Federal agencies may also require permits. The new construction of one- & two-family dwellings requires review and approval by the local Uniform Dwelling Code (UDC) authority. Additions and alterations to one - and two-family dwellings may require review and approval by the UDC authority. Permit application data is used for statewide statistical gathering on new one- and two-family dwellings, as well as for local code administration. Please type or use ink and press firmly with multi -ply form. PERMIT REQUESTED • Check off type of Permit Requested, such as structural, I-IVAC, Electrical or Plumbing. • Fill in owner's current Mailing Address and Telephone Number. • If the project will disturb one acre or more of soil, the project is subject to the additional erosion control and stormwater provisions of ch. The surveyed parcel of land and surrounding deeds use the same metes and bounds type of description and the same bearing and distance basis to create this array of parcels lying north of Old Mill Road, south of Leonard School Road and west of U.S. Highway 63. The descriptions for said parcels are written in a fashion that make it possible they were derived from a old survey map or some sort of scriveners map when they were created and partitioned. I searched for such a map, but was unable to recover any maps or old documents whether recorded or unrecorded related to these parcels. I was able to speak with Patrick Asher (owner of the surveyed parcel ) and Lawrence Baldus (owner of parcel at 11930 Old Mill Road, two parcels east of the surveyed parcel) both gentlemen are long time residents to these parcels and both said they were unaware of any old survey maps and or formal survey work which would have been done to create these parcels. The deeds for these parcels commence at the Northwest Corner of the Northeast 1/4 of the Northeast 1/4. Using the East - West measurements in the recorded deeds from the NW Corner of the NE 1/4 - NE 1/4 fits the physical location of Old Mill Road reasonably well.
